How to get a DevEUI IEEE MAC address and a AppEUI address ?

The LoRaWAN specification defines the unique address of all LoRaWAN sensors, the DevEUI, to be a valid IEEE address. It also defines the AppEUI, which may be used by the network server to identify the entity than can complete the OTA join process for the device, to be a valid IEEE address.

As the name implies, a IEEE address is allocated by the IEEE (www.ieee.org). The IEEE manages the global allocation of 64 and 48 bit binary identifiers to facilitate production of network equipment that requires unique addresses, such as Ethernet adapters, by multiple manufacturers. The IEEE registration authority does not allocate unique addresses, but rather blocks of addresses. 

As of writing, the procedure to obtain a block prefix is accessible at http://standards.ieee.org/develop/regauth/oui/

Several block sizes are available:
MAC Address Block Large (MA-L) : 24 bit prefix. Such prefix  is also called Organisational Unique identifier (OUI).
MAC Address Block Medium (MA-M) : 28 bit prefix
- MAC Address Block Small (MA-S) : 36 bit prefix
